HexEx: Robust Hexahedral Mesh Extraction

HexEx is an algorithm for extraction of hexahedral meshes from a given parametrization of a tetrahedral mesh. It is robustly detects imperfections of the parametrization and is able to meaningfully extract all hex meshes in the presence of flipped or degenerate tetrahedra.
